Dr. Harindu Wijesinghe (MBMS, MD, MRCP) is a Consultant in Rheumatology, Rehabilitation and Sports Medicine. He returned from Australia after a fellowship at Concord Hospital in Sydney, and the New South Wales Institute of Sports Medicine. His expertise is in management of Arthritis, neck pain, back pain, sports injuries and other soft tissue injuries.

In an interview with the Sunday Island, Consultant Rheumatologist and specialist in sports medicine attached to the Nawaloka Hospital, Dr. Harindu Wijesinghe spoke on multiple causes of pain and management strategies. Early diagnosis key to battling painful disease

What motivated you to become specialised in rheumatalogy?

As a rheumatologist, my motivation to specialize in rheumatology stemmed from a deep interest in the complexities of autoimmune diseases and musculoskeletal conditions. Also, my interest in sports and sports injuries.

I was driven by the challenge of diagnosing and treating these conditions to improve the quality of life for my patients.

Why did you choose to specialise in pain and rheumatalogy?

I choose to focus on pain management in rheumatological condition’s because I believe that managing pain is a crucial aspect of treating rheumatic conditions. By focusing on both pain management and rheumatology, I aim to provide comprehensive care that improves the physical and emotional wellbeing of my patients. If you’re treated early and respond to the drugs ,you stand a good chance of getting back to where you were.

What do you think are some of the most important issues in rheumatalogy?

Some of the most important issues in rheumatology include early diagnosis and treatment of rheumatic diseases, managing chronic pain effectively, and staying updated on the latest research and treatment options in the field.

What are some of the non medicinal methods you use to reassure patients who are experiencing discomfort?

Non-medicinal methods are as important as drugs and we use them extensively. We reassure patients and provide education about their condition, offering lifestyle modifications such as weight loss , muscle strengthening plans , exercise and stress management techniques.

How do you define success as a rheumatologist

I define success as a rheumatologist by the improvement in the quality of life of my patients. Success is achieved when patients experience reduced pain, increased mobility, and better overall health outcomes under my care.

What do you think due some of the most common rheumatic issues and why?

Some of the most common rheumatic issues include osteoarthritis, rheumatoid arthritis, lupus, gout, and fibromyalgia. These conditions can arise due to a combination of genetic, environmental, and lifestyle factors. Also soft tissue injuries and sports injuries like tennis elbows , shoulder rotator cuff , knee injuries , heel and Achilles problems are also common .

What is your primary approaches to pain management for patients with chronic conditions?

My primary approach to pain management for patients with chronic conditions involves a multidisciplinary approach. Most important is properly diagnosing the condition and how it affects the patient’s daily activities. After this treatment includes a combination of medications, physical therapy, occupational therapy, counseling, and lifestyle modifications to help patients manage their pain effectively.

What advice do you give patients with autoimmune disorders who are considering pregnancy?

For patients with autoimmune disorders considering pregnancy, my advice would be to consult with their rheumatologist and obstetrician to develop a comprehensive treatment plan. It is important to ensure that the autoimmune condition is well-managed before and during pregnancy to minimize risks to both the mother and the baby. Regular monitoring and communication with healthcare providers are key to a successful pregnancy for patients with autoimmune disorders.

Madonna’s free Brazil concert at Rio’s Copacabana beach attracts more than 1.6 million fans

Huge crowds attended Madonna's performance at Copacabana beach (BBC)

Madonna put on a free concert in Rio de Janeiro on Saturday night with throngs of fans flocking to see the pop idol on Brazil’s famous Copacabana beach.

“Rio, here we are in the most beautiful place in the world,” she declared, as the performance got under way.

The show was the final stop on the 65-year-old’s Celebration Tour, celebrating her 40 years in music.

More than 1.6 million fans were estimated to have attended the show.

“Are you ready?” Madonna posted on her Instagram account seven hours before the show.

Crowds of fans were seen dancing along to Madonna’s songs played from speakers on the stage, hours before the American superstar arrived.

In what many have called a “historic” show, Madonna impressed the audience with multiple set and costume changes and light displays.

The signer performed some of her greatest hits, including Nothing Really Matters, Like a Prayer and Vogue, and was joined on stage at one point by Brazilian singer Anitta.

There were giant screens located along the beach, to ensure the largely Brazilian crowd was able to see the event unfold. Some watched from apartments or hotels, others from boats.

There was also a large police presence to ensure the safety of all those in attendance.

T-shirts, souvenirs and billboards with Madonna’s face decked the neighbourhood as excitement built ahead of the concert.

Since she burst onto the UK charts with Holiday in 1984, Madonna has scored another 71 hits, including 13 number one singles.

Some, like Vogue, Like A Prayer and Ray of Light, are era-defining anthems. Others, like Live To Tell and Don’t Tell Me, are beloved fan favourites.

Fans had been anticipating Madonna’s arrival in Rio de Janeiro (BBC)

Last year, the superstar was found unconscious in her New York apartment in June and rushed to hospital, where she received treatment for a serious bacterial infection.

But she has since returned to full strength and her tour has been marked by her characteristically energetic performances.

The Rio concert was the final touch on a tour made up of 80 shows across Europe and North America.

Pakistan’s National Day Reception held in Colombo

The Islamic Republic of Pakistan’s National Day was celebrated at the BMICH Colombo recently. The High Commissioner hosted a largely attended reception. The formalities of brief speeches and the National Anthems of the two countries was followed by mingling with distinguished guests. There were many men in smart uniforms and dark lounge suits with women bringing glitz and glamour to the venue in Shalwar suits and sarees.

The High Commissioner of Pakistan H.E. Faheem Ul Aziz HI (M) in his message on the occasion said that Pakistan and Sri Lanka continue to maintain their traditionally close and mutually beneficial relations, based on mutual respect and trust. He said that the strength of the Pakistan – Sri Lanka relationship lies in diversified engagement in many fields such as trade, defense, science, culture, and education. H.E. also extended sincere greetings and best wishes on behalf of the government and people of Pakistan to the government and people of Sri Lanka.

In his remarks, the Chief Guest of the event, Nimal Siripala de Silva, Minister of Ports, Shipping and Aviation of Sri Lanka expressed satisfaction at the trajectory of bilateral relations while identifying the areas of ongoing cooperation between the two countries, the chief guest hope that the bilateral ties will be further strengthen in the years ahead. The Hon. Minister also shed light on the centuries-old relationship dating back to the golden era of Gandhara and Buddhism between the people of the two countries.

A documentary showcasing the landscapes, business potential, historical footprint, and culture of Pakistan was displayed for the guests. An exhibition showcasing the traditional items and handicrafts of Pakistan was also part of the event, which was of special interest for the guests. The participants also enjoyed traditional Pakistani music and cuisine.
